HOBIC - A First for Home Care

Bayshore and MedShare have been working together to design and implement an electronic clinical documentation solution for their visiting nurse program.  The solution is based on MedShare eMotion software running on Motion Computing tablet computers and went live in the fall of 2009.  MedShare “Smart Charts” are used to provide an electronic version of Bayshore’s nursing forms.

As Bayshore was going live, an opportunity was presented to implement an electronic version of the HOBIC Assessment form, using the MedShare Smart Charts.  The collection of of standardized clinical outcome measures provides valuable information for the nurses and Bayshore on how well the clients are progressing towards their discharge goals.

Together MedShare, Bayshore and HOBIC developed a real-time integration between MedShare’s EHR repository and the HOBIC database which is housed at ICES.  The assessment data now flows seamlessly between the systems.  Nurses in the field are able to access online HOBIC Reports to review client outcomes over the episode of care.

Helen Holmes of the HOBIC team summed it up in an email to the project team, “This is the first Home Care site live with HOBIC – this is a big milestone for the Program”.

MedShare’s design philosophy is better information enables informed decision making and improved health outcomes.  It is very satisfying to see the HOBIC measures implemented by Bayshore nurses in the community to support and enhance the delivery of care.

Bayshore/MedShare to present at OACCAC Annual Conference

Bayshore, MedShare to present "Implementing Electronic Clinical Management Systems: Creating the Foundation for Integrated Client Care" at the 2010 OACCAC Annual Conference

Bayshore, with support from MedShare, implemented an electronic clinical management system in community care settings by putting tablet computers into the hands of the visiting and shift nurse teams providing care in the NSM CCAC. The information interface provides a longitudinal electronic health record repository linking the interdisciplinary team, branch and CCAC.  Teams are able to access health information, CCAC referrals, physician orders, clinical decision supports, care plans, and care documentation tools.  This allows the measurability of severity and evaluation of client outcomes by the care team.

This presentation demonstrates the results of the implementation phase of this project, including evidence for greater consistency in clinical documentation, enhanced communication with interdisciplinary team members, outcome measurement and improving adherence to best practices.

eMotion Hazel's most significant offering is a completely revamped synchronization technology. We now provide a much simpler solution, and will be easier to extend to suit future needs. Most importantly, it is far more reliable than previous versions. MedShare's objective with Hazel was to ensure synchronization worked well under any level of network connectivity, with data integrity a key priority. Thor release now available!

We continue to improve MedShare HC, based on valuable client feedback and emerging industry trends.  We have acted on our client input by building out a series of enhancements to improve the client management, customer management, intake, time entry, billing, payroll, and dashboard processes.  By growing our administrative foundation, we persist in our efforts to improve data accuracy and increase productivity back in the office, while providing the groundwork for our mobile solutions.

Chris has worked in the IT industry for 17 years, during which he's led development efforts to create highly reliable, scalable, and available software across a variety of markets. With a deep interest and past experience with health care, Chris is committed to delivering world-class solutions to MedShare customers. Chris is involved in managing and growing the technical assets of the business and provides leadership for the development team.
